Doctor Information
The CEO is the primary point of contact for any queries by medical or dental practitioners wishing to be credentialed to practice at Waikiki Private Hospital.
Applications forms can be obtained online or by contacting (08) 9550 0222.
The following documentation is required:
- Completed application form
- Credentials (primary/specialist degree) original or certified copy
- Current Certificate of Registration to practice in the state of WA
- Current indemnity insurance membership original or certified copy
- A current CV
